Hematuria/Nephrolith

Hematuria/Nephrolith

Hope is a 7 year old female Pit Bull with a history of chonic occult hematuria. No signs of LUTD and urine was

clear of imflammatory cells and bacteria. No culture was done at this time.

X-rays are clean.

I did an ultrasound. The bladder and urethra looks clean. I did not see any obvious issue with the ureters.

I would like your opinion on this region of calcification in the L kidney. Does not appear to shadown

Is this a nephrolith? If so- it be the cause of the hematuria noted.

No real shadowing so not

No real shadowing so not convinced that it is a nephrolith but coukd be focal area of inflammation that could be causing the hematuria – any possiblity that there is pathology in the genital tract causing the hematuria?

The shadowing of stones is

The shadowing of stones is often angle dependent and may not shadow if obscured by the fibrous component of the renal pelvis and fat so would need a video to know here sweeping through different angles. Hematuria and stones really depends if a stone already moved or is moving within the kidney. Then there are blood calculi that dont shadow but can obstruct as well.

If there was a hemturia episode and  finished then I would suspect a stone was passed…. patients often are “ADR” for a day or two when that is happening then they are back to normal once moved on through. Berent et al did some nice work on stone size for passage and was up to 4 mm in a viable ureteral system but i dont recall it being patient size dependent so I go by up to 4 mm in mid/large dogs and 3 mm in cats and small dogs but Im sure this varies on many factors including the sonographic presentation and hydration.
