Prepuce swelling

  • 1 year old Lab, castrated 4 days prior (unsure how procedure went), no history of trauma post op
  • Presentented with preputial swelling and pain, can urinate, has diarrhoea
  • Scrotum and surgical site is unremarkable but severe swelling cranial to this and around prepuce
  • Subcutaneous fluid – haemorrhage/oedema?
  • Caudal abdomen has hyperechoic/reactive tissue around colon and bladder, no free fluid
  • What do you think has caused this?

    • Anytime I have seen a complication/bleed following castration the scrotum is severely swollen and then the prepuce swells.

I have seen this happen a few

I have seen this happen a few times, once to me and two that I followed up on. I think it’s a similar situation to a scirrhous cord in the equine world, basically an infection/granuloma of the cut end of the cord. In the case that was mine, after some research, I felt it was partially due to leaving too much tissue between the two ligations (I typically do a miller’s proximally and encircling distally). I never had to go in abdominally, but I did have draining tracts develop in the inguinal region requiring drain placement in the inguinal SQ both in my one case and then in a colleagues. Obviously they were treated medically at the same time. The third case resolved with ice, antibiotics, NSAIds and time, but no surgical intervention. 

All three cases ended up resolving without further complication. I hope that helps?

The swelling around the

The swelling around the prepuce has worsened and the left inguinal region is quite inflamed so it may need drainage as you have seen. Thanks again.

That is exactly the

That is exactly the progression that I’ve seen in my cases. Keep me updated! Keep scanning it! In the ones I dealt with, it became quite apparent that abscessation was occurring and warranting a drain. Dog much more comfortable after drain placement.
