Hepatic Abscess

Little is a 14 year old Himalayan cat presented 1 month ago ADR with palpable cranial abdominal mass.

Labs showed Slight neutrophilia w/ 8% bands & some liver enzyme elevations.

U/S: Cystic mass in right cranial abdomen, appears to be within hepatic tissue. GB slightly enlarged and prominent, slightly tortuous bile duct.

Drained 8 ml pus and instilled 20mg Baytril.

C&S Pseudomonas a.

Little improved and on recheck today was almost back to normal with some weight. The scan shows the cyst is filled up again, I’m assuming (hopefully) not an abscess or at least much less infected given the cat’s improvement.

The owner is reluctant to pusue surgery, so I’m planning on re-draining and possibly instilling some more baytril.

Anything else I can do to help this out…or should I be thinking of something else here other than cyst/abscess.

Cool case and images. Needs sx regardless as there are obstructive biliary stones (follow the gb to cd to cbd slowly through the videos) to address and left lobectomy to remove the abscess. i dont see anything neoplastic but carcinomas can abscess as well.
