Hmm I hate these cases…

Hmm I hate these cases… these are non specific renal changes with thickened corticies and some loss of cm junction definition. The cortex to medullae ratio is excessive. Needs a renal Bx. Immune mediated renal usually dumps protein. Are adrenals flat any chance for addisons typical or atypical?

Been screened for atypical addisons. Results about 1.8. Most atypical addisons I have seen are closer to <0.2

I did not post images of the adrenals – but they did measure normal in size. I will post them now.

Blood Elisa testing is positive for Lepto sp

Real PCR of th blood is negative but Real PCR of the urine was positive. No history of vaccination- but this was a rescue dog. I did start him on Doxycyline to be safe.

The other issue is the mild hypoalbuminemia. If no protein in the urine we are left with liver disease (no indication of this) or protein losing enteropathy (most likely) although the bowel looked OK to me.

At 1.8 I would do a full stim

At 1.8 I would do a full stim especially with unexplained low albumin because addisons does that… may be emerging addisonian. May have 2 things going on as well. Anything under 2.0 is suspect.
