– 12 year old Miniature Schnauzer
– Labs done because dog has increased it’s water consumption noticabley. ALP 433 U/L, ALT 227 U/L, Glucose 198 but nomral Fructosamine, USG 1.024
– Ultrasound revealed liver nodules (FNA’s – vacuolar hepatopathy), one lymph node enlarged near kidney (FNA – mild lymphoid hyperplasia), and adrenals measuring: R = 2.65 x .55 and .61cm, L = 1.92 x 0.65 and 0.52 cm
Question: Is it indicated to test for Cushings with adrenals this size and these labs?

A small % of PDH dogs will
A small % of PDH dogs will have normal adrenals on US… around 10% or less last I checked. If USG is dropping < 1.020 then I would do an ACTH stim and BP or run a full adrenal panel U of Tennessee but this test still has some controversy with it but if the patient looks endocrine and the tests support it then treatment would be warranted. Perceivable PUPD is usually < 1.020 so be sure there isnt pollakiuria instead caused by a uti or similar… salty diet maybe..?

Is the patient polyphagic
Is the patient polyphagic and/or polyuric as well? With the age should also consider early kidney disease.
