TCC and splenic mass

TCC and splenic mass

– 10 year old FS German Shorthair Pointer with history of hematuria

– two weeks ago my associate performed  u/s guided cystocentesis (yikes) and didn’t seen anything

– urine culture was negative blood was present

– full abdominal scan shows proximal urethral  mineralizing mass with positive colour flow on Doppler prime DDx: TCC

– no renal hydronephrosis, medial iliac LN’s identifiable but not enlarged at this stage

– 10 year old FS German Shorthair Pointer with history of hematuria

– no renal hydronephrosis, medial iliac LN’s identifiable but not enlarged at this stage

– a splenic mass with a complex echotexture distorting the splenic capsule was also found DDx: hemangiosarcoma, lymphoma, mast cell, histiocytic sarcoma, benign hyperplastic mass

– went to perform u/s guided suction biopsy to find that I could palpate the mass at the urethral papilla so was able using an otoscope and alligator forceps to get some fairly decent biopsies (results pending)

– performed FNA’s of the splenic mass (results pending)

I think the urethral lesion and the splenic lesion must be two different disease processes? Any thoughts?

Reported metastatic sites for TCC are bone, joint, lung, skin, eye. Thus most likley two seperate lesions, although can never be 100% certain with neopalsia.

That just goes to show the value of a complete exam including a full ultrasound and chest x-rays.

Recently had a TCC with lung mets.

great image set… I personally can’t recall any of our TCCs in the ugelab study of 160+ now ever metting to the spleen so if it does its likely rare. Likely 2 different pathologies but coupld splenectomy (if solitary mass 50% of these are benign ) and ugelab the urethra at ridgewood vh ridgewood NJ. Screen the hearty for pc effusion and right auricular mass as well and lung rads of course.

Histopah confirmed TCC and cytology was of low cellular yield showing only evidence of extramedullary hematopoesis. The owner has declined oncology referral and further treatment other than palliative piroxacam.

Ridgewood, a place in Chicago

Ridgewood, a place in Chicago and TAMU are the only ones I know of but there is an abyss of case load difference … at RVH we have done > 160 patients at this point since we developed the procedure in 2003. Dr Cerf does them himself there with his technical staff. I don;t mean to push to one faciltiy or the other but experience is very very important here regarding the efficiency and effectiveness of the ablation.
