True and complex renal cysts in a 9 year old Yorkie

  • 9 year old aysmptomatic MN Yorkshire Terrier presenting for senior wellness exam
  • rDVm saw prominent spleen, prominent liver, and prominent heart on radiographs
  • Normal echo and normal abdominal US exam except for multiple right renal cysts, one with an echogenic center
  • My primary differentials are benign congenital renal cysts, benign acquired renal cysts, potentially infected renal cyst, and lastly emerging neoplasia. 
  • I have recommended urinalysis and culture (CBC and chem profile are both normal)
    • 9 year old aysmptomatic MN Yorkshire Terrier presenting for senior wellness exam
    • rDVm saw prominent spleen, prominent liver, and prominent heart on radiographs
    • Normal echo and normal abdominal US exam except for multiple right renal cysts, one with an echogenic center
    • My primary differentials are benign congenital renal cysts, benign acquired renal cysts, potentially infected renal cyst, and lastly emerging neoplasia. 
    • I have recommended urinalysis and culture (CBC and chem profile are both normal)
    • What is the likelihood that this is neoplastic and how aggressively would you manage this?  FNA now or recheck sonogram on 1-2 months?

I would recommend an FNA now,

I would recommend an FNA now, as neoplasia is always a possiblity and if so removal of the kidney sooner than later is benificial. If negative then repeat the US 4-8 weeks time as well as the FNA.

I’m thinking polycystic

I’m thinking polycystic kidneys but culturing the cysts will likely be more rewarding. Doubt any cancer here. FNA of the hyperechoic nodule is what i would do regardign the parennchyma as its a bit odd.
