– 10 year old Persian with multiple episodes of syncopy.
– I have not spoken to the owner directly but seems to occur during sleep which is unusual, wakes up and seems as if retching and then faints.
– HR 156 initially no murmur with intermittent periods of an arrhythmia. Following scan (and 0.2mg/kg butorphanol + 0.02mg/kg acp im) HR regular with very soft murmur audible both sides of thorax.
– I do not see significant structural disease, I would just like another opinion please?

There seems to be some dynamic obstruction of the RVOT – this could be responsible for the murmur. Obviously, the septal bulge does not have any hemodynamic consequences (does not cause obstruction) at this time. There is also no sign of imminent or present CHF
Yet, there are arrhythmias visible on the video clips – a Holter recording would be helpful. If not feasible, a 5 min rhythm strip could as well provide some additional information. If the syncope is of cardiac origin, it has to be due to arrhythmia in this case. Cardiac syncope due to arrhyhthmias is very rare in cats and sometims a challenge to diagose.
Could be a case for Sotalol, is my gut feeling.
