Mass on kidney

-6 – 8 year old FS DSH

-recent history of anorexia, fever

-bloodwork shows neutrophilic leukocytosis

-not azotemic

-FIV/FeLV negative

-not sure if USG was performed

 

Questions:

-What are the most likely ddx for the mass attached to the right kidney?

-The left kidney looks like it has adhered fat to the capsule but I don’t appreciate a lot of morphologic changes aside from a significant size difference compared to the right side.  Any comments?

-I only noticed the biliary stasis after reviewing my images; I do not see a cause for anorexia in the clip of the liver/BG/stomach/duodenum…do you?

If you look just dorsal to

If you look just dorsal to the rk nodule there is a capsular retraction and that nodule is secondarily expansive while the pericapsular fat pattern indicates inflammation in that region. I am betting on infarct and non neoplasia here… infectious disease need to be investigated and a 25g fna of that nodule to ensure just inflammatory. Occasionally LSA can start up this way hence the need for fa. But early/recent infarct is my best guess with secondary focal swellling as the cortical parenchyma is trapped and focally expanding from the infarct if that makes sense. If this is a recent infarct it wioll organize over time and fibrose if its LSA then it will continue to expand but best to needle now and be on top of it proactively.

What type of infectious

What type of infectious disease are we looking for?  Toxo? The owners already said they will not allow FNA or many more diagnostics of any kind.  What kind of investigation for infectious disease do you suggest?

Do you agree the LK also is affected?

Any comments on the clip of the stomach, etc?

toxo?? bartonella?? likely

toxo?? bartonella?? likely idiopathic but if infectious comes up may have sonmething to shoot at. Try to culture the urine ideally. Stomach looks fine to me maybe a little muscularis hypertrophy but empty and nsf otherwise.

Thanks!  Cat was doing better on empiric antibiotics so I think we’re on the right track.
