Liver mets from adrenal pheo

  • Had a 11 yo FS Lhasa with probable Cushing’s
  • Odd looking liver in a patient with Cushings & now a probable pheo with phrenic vein invasion & CVC invasion
  • Liver has large micro-cystic masses with some degree of encapsulationIs this a met from the pheo (have never seen one this big) or a hepatoma, cyst-adenoma??

From its size unlikely to be a met and more likely a hepatoma or cyst-adenoma – but should base it on FNA cytology as it can change the prognosis some what.

I agree. What makes you think pheo if the dog is cushingoid Tomie? fna on this liver mass should give an idea as pheo cells come up neuroendocrine tumor and hepatoma and hepcell carcinoma lok completely different cytologically.

Pheo I think because of the
Pheo I think because of the phrenic/cvc invasion & dog is hypertensive. Owner declined FNA of liver mass. So can it also be adrenal carcinoma (no mineralizations were seen) & not a pheo? When things start invading the phrenic vein and/or the CVC, is it a given that it is neoplastic? BTW, dog’s ACTH was positive.

Invasive adrenal tumors can
Invasive adrenal tumors can be pheo or adenoca. Both cushings dogs and pheo dogs can be hypertensive so we can’t say pheo unfortunately because of hypertension. Since only a small percentage of pheos mineralize…0-10% according to the literature, I would put my bet on adenoca if we play the odds especially if he is pupd and + lddst or acth stim for cushings.
