left atrial rupture in a maltese dog

Just in case you haven’t seen sth like this yet:

A 12 yrs old maltese dog presented for dyspnea and acute weakness. He has already been on Pimobendan, Lasix, Spironolactone and Benazepril for 9 month for severe mitral valve disease that has already been decompensated.

Look at the clips: You see a severe pericardial hematoma formation. It got worse during the exam. A hematoma in the pericardial sac can only develop due to severe and acute bleeding. If the dog survives the episode, the hematoma will resolve.

Re this one: The patient survived the initial bleeding period but died after 2 days.

I agree, this happens very rarely. but if you see once, you will never forget how it looks like on echo 🙂

How do you convince yourself

How do you convince yourself that this is a clot and not a mass?  Color flow Doppler?  Look for wall attachment?  I have had a couple of MVI dogs lately with mild pericardial effusion, but no visible clots.  Both dogs were stable at the time of their exam.  How do I differentiate between pericardial effusion from MVI/CHF and pericardial effusion from left atrial rupture?

The suspicion comes when its a chronic valve dog has this issue as they rarely get cardiac tumors. cardiac masses usually happen wiht large breed dogs re hsa and brachycephalic dogs re ao body tumor…. whole chapter on this on “the curbside guide” plug plug (http://sonopath.com/products) coming out in the next couple of months… so in cases of chronic vallve disease usually wiht mv prolapse or flail leaflet in my experience and pc effsuion thats very echogenic like this one as its a recent bleed then its LA rupture til proven otherwise. Also it fills on the left side first with LA rupture whereas the right atrial masses fill on the right first then spread to the left. I don’t think I have ever been able to exactly see the tear on these directly have you Peter? i know Peter is on vacation but maybe he can respond when he gets back.
