The emerging Gb mucocele

So  get asked a lot how do you define an “emerging GBM.” So I was reading through cases this am and came acorss this one.

GB mucocele is defined variable as Gb overdistention with striating bile…then there are the inflammatory descriptions. But to call a mucocele a mucocele I need to have visible oversitention and no movement of the bile. This particular case I called an “emerging” mucocele and “emerging” is a great buzz term to stimulate the owner for a recheck to be done by the way… watch their eyes light up when you say “eeemeeerging….”:)

This Gb has overdistention which will occur with fasting but the bile is visibly striating  and immobile. The overdistention is not complete in this case. Not if this dog were anorexia with driving LEs then this cvould be clinical. But this dog is mildly subclinical with minor alt and moderate sap elevations so its an “emerging GBM” and i tx this baytril metro x 10 days and actigall 8 weeks and rec a GB motility study ( http://sonopath.com/resources/interventional-procedures) if they wish but the GP needs to watch for rising LEs and progressive anorexia.
