DMVD vs Endocarditis

Hello everyone,

 

Benji is a 12yo MI stray dog, never went to the vet (!), presented with hematuria, feve, anorexia and a loud heart murmur (both sides of the thorax)

In the abdominal U/S there are renal changes (cortical infarctation, loss of cortico-medullary definition, irregular silhouette, subcapsular fluid and regional reactive lymphadenopathy).

In the echocardiogram I see what seems to me a severe vegetation on the mitral valve, but as I am a bit inexperienced I’m not sure if this could be a severe form of degenerative mitral valve disease.

This can just be degentrative

This can just be degentrative valve disease and not infectious but given the fever endocarditis is possible. Try Baytril Clindamycin and then recheck the echo periodically. Blood culture ideal but they often are negative. I would scan the abdomen looking for sources such as pyelonephritis and pancreatic abscess and such.

If you take blood for culturing, arterial blood might be more sensitive. I would take both arterial and venous blood before giving antibiotics. And I would take urine samples as well for culturing. What about the aortic valve?

Standard antibiotics for endocarditis in Europe is Gentamycin/Amoxocilline-Clavulanic acid. I have also used Piperacilin/Tazobactam with success.

Benji was already on antibiotics when I scanned him (enrofloxacin), but now we’ve added ampicillin. At this point I don’t know if the owner wants to pursue any further testing but l’ll still keep you updated on the outcome.

Unfortunately Benji was euthanised yesterday due to worsening of the renal function (bilateral pyelonephritis suspected).

I hope we have better luck in the future…
