A 7-year-old SF Pug dog was presented for evaluation of hematuria and chronic urinary tract infections. The only abnormality on urinalysis was isosthenuria.

DX

Urinary bladder sand and debris, flattened adrenal glands

Sonographic Differential Diagnosis

Urinary bladder sand and debris. Minor urethral sand. Urinary tract infection is likely, combined with uroliths. Addison’s disease is possible given the flattened, isoechoic adrenal glands.

Image Interpretation

The urinary bladder in this patient presented a mild amount of sand and debris that continued into the deep pelvic urethra. The pelvic urethra was imaged approximately 4.0 cm past the cystourethral junction.There was no evidence of significant bladder wall thickening. A small calculus within the urethra measured 0.14 cm. The adrenal glands were slightly flattened in contour and were isoechoic to surrounding tissue. The right adrenal gland measured 1.45 x 0.37 cm. The left adrenal gland measured 1.56 x 0.23 cm.

Outcome

Cortisol was 5.1. The patient had an episioplasty.

Comments

If any history of potential Addisonian type symptoms are noted in a patient with flattened and isoechoic adrenal glands, then baseline cortisol and/or ACTH stimulation testing is warranted.

Clinical Differential Diagnosis

Bladder pathology – chronic bacterial cystitis, uroliths, neoplasia. Renal pathology – pyelonephritis, renoliths, neoplasia.
