Urethral and prostatic mass with iliac lymph node involvement in a 9 year old MN Dachshund

A 9-year-old NM Dachshund was presented for evaluation of dyschezia and dribbling. On physical examination, non-painful prostamegaly was present. Survey radiographs showed a calcified area within the pelvic canal.

Image Interpretation

The prostate was enlarged, hypoechoic with multiple, focal hyperechoic areas of mineralization that continued into the preprostatic urethra. Mild, periserosal inflammatory pattern was noted around the prostate.

The bladder itself was unremarkable with slight sand accumulation in the bladder.

The iliac lymph node was enlarged, hypoechoic and focally mineralizing. This is strongly suggestive for metastatic disease. The lymph node measured 1.86 x 1.2 cm. 

DX

Urethral and prostatic mass.

Outcome

Iliac lymph node involvement. This is strongly suggestive for metastatic disease. Regional inflammation.
Ultrasound-guided FNA is recommended for further definition. Palliative therapy with urethral stents could be considered in this patient as well as chemotherapeutic intervention. Traumatic catheterization or FNA would be recommended of the iliac lymph node and prostate for confirmation.
