Transitional cell carcinoma of the bladder in a 12 year old FS Miniature Pinscher

The patient is a 12 year old spayed female Miniature Pinscher with increased liver enzyme levels in the past two blood screens. The dog has a previous history of hematuria.

DX

Histopathology revealed transitional cell carcinoma.

Sonographic Differential Diagnosis

Dorsal bladder tumor, appears resectable. Alternatively, ultrasound-guided laser ablation could be utilized in this case and may be ideal if micro-metastasis is noted within the pelvic urethra. If any metastasis were visualized by the scope, they could be treated at the time. Otherwise, dorsal partial cystectomy of the bladder wall would be recommended. Unfortunately, the surgeon will have minimal tissue with which to work.

Image Interpretation

The urinary bladder in this patient contained a dorsal wall mass that measured 2.18 cm at the base of the mass and 1.56 cm at the mid-body of the mass. This mass encroached upon the ureters and ureteral papilla, but did not obstruct the flow of urine. The mass extended caudally approximately 0.8 cm from the right ureteral papilla. This may prove to be resectable. Significant vascularity was noted with focal areas of mineralization, which is strongly suggestive of transitional cell carcinoma.

Clinical Differential Diagnosis

Hematuria – renal disease (neoplasia, reno/ureterolith, pyelonephritis, telangiectasia), bladder disease (neoplasia, cystitis, urolith). Elevated liver enzymes – vacuolar hepatopathy, reactive hepatopathy, nodular regeneration, neoplasia, toxins, infectious (bacterial/fungal), chronic-active hepatitis.

Histopathology revealed transitional cell carcinoma.
