Suspected multicentric lymphoma involving small intestine, kidney and bladder in a 9 year old MN DSH cat

Case Study

Suspected multicentric lymphoma involving small intestine, kidney and bladder in a 9 year old MN DSH cat

This 9 year old MN DSH cat has a history of weight loss, hematuria and loss of appetite. WBC 40,000. Suspect intestinal mass.

This 9 year old MN DSH cat has a history of weight loss, hematuria and loss of appetite. WBC 40,000. Suspect intestinal mass.

Image Interpretation

The urinary bladder presented concentric wall thickening with loss of mural detail. The right kidney presented a relatively uniform cortical hyperechogenicity when compared to the renal medulla, spleen and liver. No overt masses were noted. Some mildly echogenic debris was present in the medulla.  The left kidney presented a hypoechoic mass that measured 1.77 cm and was deriving from the dorsal contour with adjacent nodule in the caudal aspect of the renal cortex.  The liver was mildly hypoechoic, slightly irregular and swollen.  An intestinal mass was noted in this patient with complete loss of detail and periserosal inflammatory pattern. The mesenteric lymph node was enlarged and measured 1.7 x 0.76 cm.  Free fluid was noted coalesced in the cranial abdomen.

DX

Left renal mass, intestinal mass, diffuse bladder thickening

Outcome

Suspect multi centric lymphoma involving the small
intestine, left kidney and bladder. Other possibilities include intestinal carcinoma,
interstitial cystitis and metastatic or primary renal neoplasia. Recommend FNA of the
bladder, left kidney cortex and intestine with expectations towards multi centric
lymphoma or similar. Very guarded prognosis. Hepatic FNA could also be considered.
