An 8-year-old NM beagle mix dog was presented for weight loss. CBC showed lymphopenia, with normal serum biochemistry. Low T4.

DX

Splenic extramedullary hematopoeisis, suppurative gastritis

Sonographic Differential Diagnosis

The appearance of these images is consistent with gastritis or potential infiltrative disease such as carcinoma or lymphoma. Depending on cytology results, splenectomy and gastric biopsies may be needed.

Image Interpretation

The spleen presented multiple echogenic nodular changes with capsular expansion and disruption of architecture. Each measured approximately 2cm. The stomach in this patient presented some minor variable gastric wall thickening in the greater curvature with associated lymphadenopathy.

Outcome

The patient was given carafate, metronidazole, and amoxicillin, and is doing much better.

Clinical Differential Diagnosis

Weight loss – husbandry (poor diet, food competition), neoplasia, gastrointestinal disease (oral disease, Helicobacter gastritis, IBD, neoplasia, helminths)

Sampling

Ultrasound-guided fine-needle aspirates were performed on the hypoechoic portion of the wall. Cytology of the gastric wall was suggestive of suppurative inflammation. Cytology of the spleen revealed extramedullary hematopoiesis.
