The patient is a canine mixed breed, 11 years old, neutered male, with a history of respiratory distress. The patient has a grade IV/VI left sided heart murmur.

DX

Mitral valve regurgitation with left atrium enlargement. Stage C1 valvular disease.

Sonographic Differential Diagnosis

Mitral valve regurgitation with left atrium enlargement. Stage C1 valvular disease.

Image Interpretation

The echocardiogram for this patient presented a left atrium of excessive size with anechoic content as did the corresponding LA:AO ratio. No evidence of “smoke” or thrombotic activity was noted. The atrial septum was deviated toward the right atrium without evidence of dilation or contraction. The cranial and caudal mitral valve leaflets demonstrated mildly vegetative contour. The left ventricle demonstrated prominent volume, normal septal and free wall diastolic thicknesses, and papillary integrity. Ventricular function was deemed adequate to hyperdynamic.

Outcome

The patient was treated with triple therapy of ace inhibitor, furosemide, and pimobendan and stabilized to normal activity within 1 week.

Clinical Differential Diagnosis

Valvular insufficiency, congestive heart failure, respiratory disease with compensated valvular disease, pain, thromboembolic disease.
