This 5 year old FS Schnauzer mix dog presented for diarrhea and weakness. History of chronic diarrhea/colitis.

DX

Splenohepatomegaly. Focal splenic mass diagnosed as histocytic sarcoma

Image Interpretation

The spleen was mildly enlarged and mildly irregular with micronodular changes, and an isoechoic mass with microcystic changes.  The liver was uniformly enlarged. This is consistent with vacuolar hepatopathy liver pattern.  A minor amount of pyloric shadowing material was noted in the stomach

Outcome

The shadowing material in the stomach may be oral medications. Recommend
splenectomy in this patient. Liver biopsy could be performed. Both lesions may be
benign and non related to clinical signs. However, the splenic changes are disruptive
enough to warrant a splenectomy. Ultrasound-guided FNA of the spleen and liver
could also be considered. The hematocrit of 15% would warrant blood transfusion
prior to surgery. Bone marrow aspirates would be indicated. Infiltrative neoplasia is
significantly possible in this case.
