A 12 year NM DSH with a history of intestinal adenocarcinoma was presented for lipoma staging and to check the abdomen for metastasis. Survey thoracic radiographs and blood work were within normal limits.

Splenic hyperplasia

Sonographic Differential Diagnosis

Undefined hyperechoic splenic nodules. This is most consistent with nodular hyperplasia, lipid deposits. amyloidosis, or residual from history of splenitis. However, fine-needle aspirates would be warranted to rule out metastatic disease given the patient history.

Image Interpretation

The spleen was normal in size. However, multiple hyperechoic nodules were noted.

Clinical Differential Diagnosis

Metastatic disease – liver, spleen, kidneys, mesenteric lymph nodes

Sampling

U/S guided FNA of the splenic nodules show marked increased hematopoiesis composed of numerous cells from the myeloid and erythroid lines and occasional cells from the megakaryocytic line. This cytology was interpreted as splenic hyperplasia. Surgical excision and histopathology to further characterize this mass may be warranted dependent on the clinical presentation of the patient.
