Splenic congestion diagnosed by FNA in a 3 year old MN DSH cat

A 3-year-old MN DSH cat, with a history of hypertrophic cardiomyopathy, was presented for weight loss and vomiting. Additional history included urinary tract infections, which resolved on Clavamox therapy. The only significant abnormality on physical examination was grade III/VI murmur. Urinalysis was within normal limits.

DX

Splenic congestion

Sonographic Differential Diagnosis

Hypersplenism is most likely; other benign causes include splenic congestion, splenitis, or lymphoid hyperplasia.
Malignancies are also possible and include the following differentials: MCT, LSA, or multiple myeloma.

Image Interpretation

The spleen was mildly and uniformly enlarged at 1.6 cm enlarged with minor scalloping of the capsule. Fine-needle aspirates were performed to rule out early infiltrative disease versus hypersplenism.

Outcome

The vomiting resolved, and the patient is doing well.

Clinical Differential Diagnosis

Gastrointestinal disease – foreign body, IBD, neoplasia, Helicobacter gastritis, dietary hypersensitivity, non-specific gastro-enteritis (toxins/protozoal/helminths, dietary indiscretion). Drug reaction. Progressive cardiac disease.

Sampling

The cytology of the spleen revealed a markedly hemodiluted spleen or splenic congestion.
