A 3-year-old FS English Bulldog with a history of chronic urinary tract infections was presented for evaluation. Urinalysis showed pyuria, hematuria, and bacteria. Abdominal radiographs were within normal limits.

Small flat adrenal glands

Sonographic Differential Diagnosis

Small flat adrenal glands. Minor chronic cystitis, no evident ectopy or structural cause of UTI.

Image Interpretation

The adrenal glands were excessively small, flat and isoechoic. This may be normal for this patient or related to potential hypoadrenocorticism (Addison’s disease). ACTH stimulation should be performed especially if any clinical signs consistent with Addison`s disease emerges such as azotemia, lethargy, or unexplained gastrointestinal signs.

Outcome

The patient was treated with a course of antibiotics for 30 days. ACTH stimulation test was not done. Follow up urinalysis showed ongoing pyuria and bacterial rods.

Clinical Differential Diagnosis

Recurrent urinary tract infections – urinary tract defect (persistent urachus or diverticulum), uroliths, inappropriate therapy (incorrect antibiotic/short course/not medicated by owner), metabolic disease (hyperadrenocorticism, renal disease).

Sampling

US guided cystocentesis for urine culture. Urine culture and sensitivity and an ACTH stimulation test were recommended. Escherichia coli was cultured from the urine.
