Severe pulmonic stenosis and right heart enlargement in a 6 year old MI mixed breed dog

A 6 year old intact male mixed breed dog was presented for dribbling urine. The only abnormality on physical examination was a heart murmur. Urinalysis was within normal limits.

Sonographic Differential Diagnosis

Severe pulmonic stenosis with right sided overload. Severe tricuspid insufficiency.

Image Interpretation

The cardiac window is difficult in this patient due to excitement and wall movement. Regardless, the right ventricle and right atrium are very enlarged at 2.5-3 times the LV and LA respectively. Spectral Doppler evaluation of the pulmonic valve reveals severely excessive outflow velocity of 6.19 m/sec. Secondary tricuspid insufficiency is also severe at 4.98 m/sec. Sedation and a more thorough echocardiogram would be ideal to rule out further defects.

Outcome

The patient was lost to follow-up.

Clinical Differential Diagnosis

Urinary incontinence – UTI, bladder trigone neoplasia, neurogenic. Heart murmur – congenital valve disease, valve insufficiency, anemia, physiologic, cardiomyopathy.
