DX

Bile Peritonitis, ruptured gallbladder mucocele

Image Interpretation

Gallbladder in this patient was severely dilated with reactive surrounding fat and striating bile, yet somewhat collapsed. This is consistent with ruptured gallbladder mucocele and bile peritonitis. Common bile duct revealed echogenic debris, yet was obscured by the inflammation and peritonitis.   Reactive mesentery was noted throughout the cranial abdomen encompassing portions of the pancreas, yet the pancreas was not the primary issue.  Free fluid was noted throughout the caudal abdomen.

Outcome

Ill-defined hepatic parenchyma around the gallbladder. Recommend plasma transfusion and immediate cholecystectomy and expect abdominal lavage. Common bile duct lavage is warranted as well. Very guarded prognosis.

Comments

The patient was humanely euthanized
