DX

right auricular mass

Image Interpretation

The vena cava and hepatic veins were dilated. This is consistent with passive congestion owing to the tamponade effect. The gallbladder was double layered owing to ascites.  The cardiac presentation revealed a mixed, hypoechoic mass that was deriving from the right auricle and right ventricular free wall. Pericardial effusion was noted. The heart was volume contracted.

Outcome

Passive congestion liver pattern. Pericardiocentesis for palliative therapy would be recommended in this patient.
Given the position and echotexture of the mass this is most consistent with hemangiosarcoma.
Poor long term prognosis. Empirical chemotherapy could also be considered.
