DX

Left renal mass and mildly enlarged spleen

Image Interpretation

The left kidney measured 6.45 cm and revealed a mass deriving from the caudal pole, which appears to be encapsulated within the renal capsule. The mass measured approximately 3.0 cm. Regional free fluid was noted as well as inflammation. This appears to be potentially adhered to the body wall. However, no overt capsular escape was noted at this time, but it appears to be initiating, as the capsular contour appears to be nebulous in many portions, especially against the body wall. The spleen was mildly enlarged. 

Outcome

This is strongly suspicious for renal hemangiosarcoma or possible carcinoma. There
was no overt evidence of metastatic disease in this patient. I recommend three view
chest radiographs if not already performed with an echocardiogram and immediate
surgical consultation or direct left nephrectomy. However, the surgeon should be
prepared for potential adhesions to the body wall. I am strongly suspicious of renal
hemangiosarcoma in this patient. The good thing is that the renal capsule appears to be
containing the pathology. However, an inflammatory pattern is noted and microscopic
escape may be occurring. Guarded prognosis.
