Renal dysplasia in a 2 year old MN Shetland Sheepdog

A 2-year-old neutered male Sheltie dog was presented for evaluation of renal disease. An inappropriate urine specific gravity was present on urinalysis, urine culture was negative, and elevated creatinine was evident on laboratory testing. The patient was also lethargic, despite a 2 week course of antibiotics, and his systolic BP was 130.

DX

Severe primary renal dysplasia with secondary degenerative disease.

Sonographic Differential Diagnosis

Severe primary renal dysplasia with secondary degenerative disease is most likely, although there is a minor potential for neoplasia. Lack of inflammatory sediment on urinalysis supports primary renal dysplasia. Poor long-term prognosis. Ongoing blood pressure monitoring and treatment for renal failure would be warranted.

Image Interpretation

The kidneys in this patient were significantly dysplastic and subnormal in size; the left kidney measured 3.5 cm and the right measured 4.0 cm. Complete disruption of the corticomedullary structure was noted with capsular retraction, irregular contour, pyelectasia, and subnormal color flow signal within the cortices of both kidneys.

Outcome

The patient was eventually euthanized.
