RADS – Peritonitis with Possible Foreign Body in a 8 year old FS Tibetan Terrier Dog

History- Bloody diarrhea and vomited breakfast this morning, with some stretching. Physical Exam: BAR, T-101.4. Moderate bloody stool on glove, mild pain on abdominal palpation. CBC and Chem WNL

DX

The radiographic findings are compatible with peritonitis localized to the right cranial abdominal quadrant. Possible underlying causes include pancreatitis with reactive gastroduodenitis and intestinal pneumatosis and necrotizing enteritis. A linear foreign body is a possible differential diagnosis. The findings of the spleen suggest reactive hypersplenism.

Image Interpretation

right lateral and VD abdomen- Osseous structures:
Within normal limits.
Intraabdominal structures:
The serosal detail was mildly reduced emphasizing the right cranial abdomen caudal to the stomach in the region of the right limb and pancreatic body. The gastric outlet angle was widened and the stomach was mildly to moderately distended with gas and rigid in appearance in all views. The pylorus presented moderate cranial displacement.
The descending duodenum was corrugated. The ascending duodenum and jejunum presented plication distal to the caudal duodenal flexure. The wall was reduced in opacity suggestive for pneumatosis.
The spleen was mildly enlarged.
Intrathoracic structures:
There was hypovolemia with underperfusion of the lung. The cardiac silhouette was within normal limits for size and shape. The lung parenchyma was within normal limits as far as included.

Comments

Immediate abdominal ultrasound is strongly advised for further workup as the discrimination of the aforementioned differential diagnoses may have a significant influence on the further management of the case.

Clinical Differential Diagnosis

Suspicious of linear FB in proximal small intestine.
