This 3 year old MN DSH cat presented dehydrated with a severely enlarged firm bladder(unable to express) and bruising on tip of penis.

Image Interpretation

lateral and VD abdomen – The serosal detail is obscured and patchy in the region of the urinary bladder neck. The urinary bladder is distended, no mineral opaque material is seen associated with the urinary bladder. A faint focal mineral opacity is seen caudal to the ischiac bone and potentially consistent with the course of the urethra.  The spleen is volume contracted.

DX

Possible urethral calculus/sand. This being dirt in the fur superimposed onto the pelvic urethra has to be considered a possibilty as well. Also nonradioopaque material – such as cystine or urate – cannot be ruled out along the lower urinary tract. • Localized effusion in the region of the bladder neck which is commonly seen with obstructive pathology of the urinary bladder

Outcome

Consider further imaging by means of abdominal ultrasound for complete assessment
of the situation of the lower urinary tract and to adjust the case management.
Obtain urine culture and sediment.
