RAD – Pulmonary metastatic and chronic airway disease in a 10 year old MN mixed dog

This 10 year old MN mixed dog was found abandoned and presented with muffled heart sounds, grade 2/6 left systolic murmur, intermittent irregular arrythmia. 

Physical exam: dehydrated, severe dental disease, intermittent left front lameness, severe hindlimb weakness, underweight, several large SQ masses with one ulcerated and necrotic. HCT 32%, hookworms.

This 10 year old MN mixed dog was found abandoned and presented with muffled heart sounds, grade 2/6 left systolic murmur, intermittent irregular arrythmia. 

Physical exam: dehydrated, severe dental disease, intermittent left front lameness, severe hindlimb weakness, underweight, several large SQ masses with one ulcerated and necrotic. HCT 32%, hookworms.

Image Interpretation

right lateral and VD thorax, lateral neck – Multiple small soft
tissue opaque interstitial nodules are noted throughout the lung parenchyma.
The lung reveals a moderate generlaized bronchointerstitial pattern with
peribronchial cuffing. focal or generalized enlargement is noted. The major vessels are within normal
limits. No vascular congestion is seen.
Severe osteoarthritis of the elbows is noted as a consequence of elbow
dysplasia. A large irregular open wound is seen caudal to the left humerus.
Multiple soft tissue opaque nodules are seen in the region of the armpit and
manubrium. A diablo bullet is seen within the soft tissues of the right armpit.

DX

Nodular interstitial lung pattern • Moderate active bronchointerstitial lung pattern. The findings are suggestive for pulmonary metastatic disease

Outcome

No evidence of congestive heart failure. Granulomatous and
fribtoic nodular lung disease are unlikely. Allergic versus infectious (viral, bacterial,
parasitic) chroic airway disease appears to be present as well. Ultimate confirmation
of the tentative diagnosis theoretically would require smapling of the lung nodules
