This 8 year old MN DSH cat had a recent perineal urethrostomy (PU) procedure and has clinical signs consistent with a urinary tract infection.

Image Interpretation

Rads of the abdomen – The kidneys are of normal size even if in the lower region of the reference range. Renal shape and opacity is within normal limits. The urinary bladder is moderately
distended and normal for position and delineation. The urinary bladder neck is well
delineated. No abnormal opacities are associated with the urinary bladder and bladder
neck region.
The perineal urethrostomy (PU) site presents mild soft tissue swelling. A well
delineated ovoid mineral opaque structure of 2 mm diameter is seen immediately
caudal to the ischiac tuberosity and 1.5 cm cranial to the PU site on the lateral view
.

DX

Status after PU surgery with possible urethral stone.

Outcome

Differentials include part of normal bony anatomy of the pelvis and dystrophic
mineralization. The position and shape of the mineral opaque structure do not match
the os penis, which can be seen in approximately 40 % of male cats.
Retrograde catheterization of the urethra is warranted to test for possible crepitus and
obstruction. If this is inconclusive a retrograde positive contrast urethrogram (at least 2
orthogonal views) should be obtained.
