This 12 year old MN Golden Retriever has a history of diagnosis of lung cancer 6 months ago,  not on any meds. Fungal titers pending. R/O neoplasia vs fungal vs other.

Image Interpretation

Rads of the thorax –  An increased perihilar soft tissue opacity is noted as well as a steep ventral dip of the carina.  The lung is well expanded and presents an overall increase in interstitial opacity dominated by a structured nodular pattern with multiple well-defined interstitial nodules overlying a miliary lung pattern.

DX

The radiographic changes are equivocal for metastatic and granulomatous – such as fungal – lung disease

Outcome

The clinical history seems to support granulomatous disease, especially if there is no known primary neoplasia in this patient. Even though CT would be ideal to further define the structural changes is would likely not add to the diagnostic value. Ultrasound guided aspiration of the pulmonary nodules (alternatively bronchoscopic sampling) should be considered instead in case the fungal titers are inconclusive. This can even be done awake or with light sedation in many
patients. Screening for a primary tumor should be considered as well if not performed yet
