RAD -Chronic lower airway disease and hypovolemia in a 16 year old FS DLH cat with chronic renal failure and IMHA

This 16 year old FS DLH cat has a history of chronic renal failure and recently IMHA.

Physical exam: very thin, slightly jaundiced, quiet. PVC 24%, TP 5.8g/dl. Received tranfusion.

DX

Allergic chronic feline lower airway disease with air trapping, possible active bronchitis and lobar atelectasis • Hypovolemia with underperfusion of the lung • Mild hepatomegaly – consistent with diffuse infiltrative hepatopathy such as vacuolar, hepatic lipidosis, (cholangio)hepatitis or diffuse neoplastic infiltrate.

Image Interpretation

right lateral, left lateral and VD thorax – The lung volume is expanded. Widening of the intercostal spaces is noted. The
diaphragmatic cupola is flat. The lumbodiaphragmatic angle is slightly expanded. The
lung reveals a moderate to severe generalized bronchial pattern with bronchial
mineralization, multifocal bronchiolithiasis and peribronchial cuffing. The right middle
lung lobe reveals a lobar sign with volume loss and shift of the mediastinum to the
right which is consistent with atelectasis. The liver reveals mild generalized enlargement with caudal rotation of the gastric axis.
The liver margins are still pointed and smooth.

Consider further definition with abdominal ultrasound and sampling
