RAD – Chronic inflammatory airway disease in a 13 year old FS Pomeranian

This 13 year old FS Pomeranian presented with productive coughing, lethargy and inappetance for the previous 2 days.

DX

The overall findings are compatible with moderate chronic inflammatory airway disease such as allergic, parasitic, bacterial, viral, irritant bronchitis.

Image Interpretation

Rads of the thorax –  A uniformly fat opaque ovoid structure (about 2 cm in diameter) is superimposed on to the subcutaneous tissue of the ventral thoracic wall cranial to the xiphoid process.  The cervical trachea is mildly dilated in diameter.  The bronchial tree presents moderate to severe generalized wall thickening. Multiple thin bronchial ring shadows are visible throughout the periphery of all lung lobes.

Outcome

• Moderate to severe generalized bronchial pattern.
• Suspected Lipoma in the mid ventral thoracic wall
• Large fat deposits within the cranial and caudal mediastinum
The increased visibility of the bronchial walls (bronchial pattern) is due to true
bronchial wall thickening, mucosal thickening, peri-bronchial infiltration or a
combination of the aforementioned.
An endoscopic examination with BAL, blood work and a faecal examination (to rule
out lung worm infection) would be ideal for further definition. A clinical trial with
empirical antimicrobial treatment, theophylline, asthma spray and inhalation may be
considered as an alternative option if the aforementioned is not available.
