RAD – Chronic degenerative joint disease (DJD) left stifle compatible with cranial cruciate ligament (CCL) rupture in a 5 year old FS Pit Bull mix

Case Study

RAD – Chronic degenerative joint disease (DJD) left stifle compatible with cranial cruciate ligament (CCL) rupture in a 5 year old FS Pit Bull mix

This 5 year old FS Pit Bull mix has a history of luxating patella. Presented with non weight bearing on left hind, moderately thickened stifle, slight positive on cranial drawer. R/O CCL rupture.

Chronic degenerative joint disease of the left stifle joint

Image Interpretation

RADs of the stifles-

Left:  Severely increased articular soft tissue opacity with cranial compression of the infrapatellar fat pad and caudal displacement of the facial planes. Moderate osteophyte formation on the cranioproximal aspect of the femoral trochlea, at the apex and base of the patella on M-L view. Moderate osteophyte formation at the lateral surface of the lateral femoral condyle on the Cd-Cr view and on tibial condyles on M-L view. Moderate osteophyte formation at fabellae (medial more than lateral). Mild caudal elongation of the tibial plateau by smooth new bone. Mild to moderate cranial displacement of the tibial plateau. Popliteal sesamoids mildly distally displaced and superimposed with the tibial plateau in M-L view.

Right: Patella slightly medially displaced and with irregular contour. Moderate osteophyte formation at the lateral surface of the lateral and medial  femoral condyles on the Cd-Cr view. Moderate osteophyte formation at the fabellae.

Outcome

Chronic degenerative joint disease of the left stifle joint with a history of patella
luxation, moderate osteoarthrosis, severe articular enlargement and moderate cranial
tibial subluxation: compatible with joint effusion and/or capsular & synovial
thickening secondary to cranial cruciate ligament failure.
Patella luxation and cranial cruciate ligament pathology should be considered also in
the right stifle as it presents moderate degenerative changes as well. Clinical
correlation is warranted.
