RAD – Chronic degenerative joint disease (DJD) bilateral stifles, moderate joint effusion in a 4 year old FS Shepherd mix

This 4 year old FS Shepherd mix dog presented with a historuy of lameness of 1 week duration.

Physical exam: left hind limb grade 1/5 lameness, toe touching, suspected positive cranial drawer, no tibial thrust noted

Image Interpretation

Rads of left stifle- A moderate articular soft tissue swellig is noted. There is a medial buttress sign. The periarticular margins of the left femorotibial and femoropatellar joint reveal a moderate amount of osteophytes. Degenerative small well-delineated subchondral bone cysts are seen as well. A mild cranial tibial thrust appears tob e present. However the assessment is limited to the degree of stifle jont flexion as this should only be evaluated with the joint in 90° flexed position

Rads of pelvis – The conformation of both hip joints is within normal limits 

DX

• Moderate chronic degerative joint disease (DJD) of the left stifle with moderate joint effusion/synovial thickening • Moderate disuse muscle atrophy of the left hind limb • Emerging degerative joint disease of the right stifle

Outcome

The most likely underlying disease is cranial cruciate ligament pathology as a consequence of the chronic DJD. Meniscal pathology and primary osteoarthritis are a potential too. Other arthropathies, such as immune mediated arthritis, are not likely.
If the clinical exmination does not allow for confirmation of biomechanical instability repetition of the tests under deep sedation/general anesthesia, ultrasonographic examination or magnetic resonance imaging may be considered for further definition as non-invasive measures (compared to immediate arthroscopy/arthrotomy).
