DX

The size, shape and mineral opacity of the multiple interstitial foci is compatible with age related incidental pulmonary osteomas/pleural plugs. There is no evidence of metastatic spread to the lung or mediastinal lymph nodes readiogrphically

Image Interpretation

Rads of the thorax –
Extrathoracic structures:
There is moderate generalized osteopenia which is likely age related and/or
paraneoplastic in origin. The 6th and 8th thoracic vertebrae are wedge shaped
hemivertebrae which is a breed specific congenital malformation that is not necessarily
of clinical significance. There is mild mid thoracic lordosis with moderate degenerative
changes including spondyloses and spondylarthroses. The 2 last sternebrae are
shortened.
Both shoulder joints and elbow joints reveal moderate and severe osteoarthritic
changes respectively.
There is a rightsided fat opaque ovoid swelling along the chest wall which is
superimposed by a positioning device.
There is generalized mammary and mamillary swelling with heterogeneous patchy
opacities throughout. Several mass lesions of varying size and heterogeneous opacity
are associated with the inguinal and mid abdominal mammary complexes.
Intrathoracic structures:
Even though the degree of lung inflation is only fair and 10 cm of fat deposits within
the chest walls had to be penetrated multifocal small angular mineral opacities are
distinctively seen throughout the lung parenchyma. None of these visible opacities is beyond 3.5 mm in diameter.
The lung reveals a moderate age related generalized increase in bronchointerstitial
opacity. Multifocal bronchial wall mineralization is noted.
The mediastinum and pleural space are within normal limits.
The cardiac silhouette is within normal limits for size and shape. The major vessels
and pulmonary vasculature are within normal limits. There is no sign of vascular
congestion.

Outcome

Actual soft
tissue nodules – such as metastases – are only visible radiographically when they
exceed the size of 5 mm. This holds true especially in this case where the low degree
of lung inflation and thick chest wall impedes the chances to detect small metastatic
nodules.
The findings of the mammary parallel the history of mammary tumor(s) and further
suggest accompanying mastitis.
There is a possible rightsided chest wall lipoma.
