A 9-year-old NM canine mix was presented for evaluation of hematuria and urinating in the house that has not responded to a course of Baytril. Red and white blood cells were evident on urinalysis.

DX

Prostatic mass strongly suggestive of carcinoma

Sonographic Differential Diagnosis

Prostatic mass, strongly suggestive of carcinoma.

Image Interpretation

A large, mineralizing, prostatic mass, with peripheral inflammation and sublumbar lymphadenopathy, measured 3.3 cm with heterogenous parenchyma. This is strongly suggestive of prostatic carcinoma. Areas of cystic change were noted. Hyperechoic, inflammatory pattern was noted with moderately positive power Doppler signal present. The prostate was relatively uniform at 3.02 x 2 cm and 3.6 x 1.8 cm in the right and left lobes respectively.

Outcome

The patient was referred to an oncologist.

Comments

FNA of the prostate is essential in this patient. Stent placement and potential chemotherapy should be considered. Guarded to poor long term prognosis.

Clinical Differential Diagnosis

Hematuria – bladder pathology (resistant bacterial cystitis, uroliths, neoplasia, granulomatous cystitis), prostatic pathology (neoplasia, prostatitis, renal pathology (pyelonephritis, renolith, neoplasia).
