A 13-year-old MN Maltese was presented due to concern over bladder issues and hematuria. Radiographs revealed NSF. Urine specific gravity was 1.027, WBC 30-50, RBC >50. Chemistry AP 614, ALT 65, spec cPL was 321.

Image Interpretation

The prostate in this patient presented a mineralizing mass that measured 3.21 x 2.25 cm with sublumbar lymphadenopathy that measured 2 x 1.5 cm with a pericapsular inflammatory pattern. The prostatic mass presented polypoid proliferation into the cystourethral junction as well. Cystic portions of the prostatic mass were also noted or this may represent dramatically proliferative tissue.

Outcome

FNA’s were performed upon the prostate without complication. Prostatic mass with cystic portions. Potential drainage of these regions can be considered to provide palliative treatment. This mass appears to invade and destroy the prostatic urethra with expansive mineralizing neoplastic tissue. No visible prostatic urethra was noted in the 4.0 cm from the cystourethral junction caudally. Stent placement is a possibility in this case. Deramaxx and Enrofloxacin are recommended in the meantime.

Clinical Differential Diagnosis

Urolithiasis, UTI, urinary or prostatic neoplasia, coagulopathy.
