The patient is a 5 month old female intact DSH cat, who presented for open mouth breathing, and hyperinflated lungs and cardiomegaly on rads.  The patient was on Azithromycin, Cerenia, and O2.

DX

Pericardial diaphragmatic hernia of the liver, with normal cardiac structure and function

Image Interpretation

A pericardial diaphragmatic hernia of the liver was noted. The vena cava was followed through the diaphragmatic hernia into the right atrium. The hepatic veins were emptying into the vena cava in the caudal thorax.

Comments

Supportive care was recommended in this patient, as well as bronchodilators and broad-spectrum antibiotics, as surgical intervention in these patients does carry significant risk.
