A 3-month-old male neutered Yorkipoo dog was presented with a history of coughing. On survey radiographs, a diaphragmatic hernia was suspected because of a nebulous appearance of the diaphragm.

Pericardial diaphragmatic hernia of the liver. Stable heart. Mildly prominent right atrium and slight tricuspid insufficiency. This is likely secondary to the diaphragmatic hernia in cor pulmonale type presentation.
The right atrium was slightly prominent in this patient, which is likely a secondary effect from the pericardial diaphragmatic hernia. Slight pulmonic insufficiency was noted. The cardiac presentation in this patient appeared largely normal from an internal standpoint. However, there was no separation between the pericardium and thoracic base as well as the diaphragm. The liver lobes appeared to involve the pericardium. The heart appears stable at this time with the following mmode measurements: FS: 37%; EF: 71%; LA/AO 1.3; RVOT 0.6; LA: 0.9; LVIDd 1.6.
No treatment was performed. The owners decided to observe the patient.
The patient is clinically stable at this point. Surgical correction would be ideal. However, this would be a delicate intervention.
