A 6-year-old FS Golden retriever with a history of eating a plastic toy a few days prior was presented for evaluation of vomiting, anorexia, and lethargy. Serial radiographs were not indicative of intestinal obstruction.

DX

Right pancreatic abscess and regional peritonitis

Sonographic Differential Diagnosis

Right pancreatic abscess and regional peritonitis. There is a potential that this is a result from penetration of foreign body. Concurrent pancreatitis elsewhere. I recommend surgical intervention in this patient for the potential of penetrating foreign structure that may still be present or possibly already evacuated leaving the abscess behind. Gastric biopsies would be recommended as well as renal biopsies given the abnormal renal structure and patient history of PU/PD. Guarded prognosis.

Image Interpretation

The right limb of the pancreas was mildly heterogenous with ill defined, surrounding fat. This is suggestive for chronic active inflammation. An abscess that measured 6.07 x 4.19 cm in the right pancreatic limb. This abscess is adjacent to the pyloric outflow.

It is a possibility that penetrating foreign body or gastric wall penetration may have caused the abscessation. A foreign structure was not seen; however, this could not be ruled out. Regional peritonitis was noted.

GIT – foreign body, neoplasia, ulceration, IBD, focal perforation with peritonitis
Pancreas – pancreatitis, neoplasia
