Pancreatic carcinoma and splenic metastasis in a 15 year old FS DSH cat

A 15-year-old FS DSH cat was presented for weight loss, decreased appetite, hematuria, and lethargy. Abnormalities on physical examination included a large irregular mass in the mid-abdomen and left-sided iridial neovascularization with a small white mass on ventral/lateral limbus. On CBC, anemia and lymphopenia were present. Blood chemistry showed mild hypoglycemia, elevated BUN, and elevated AST activity. Patient had a low T4.

A 15-year-old FS DSH cat was presented for weight loss, decreased appetite, hematuria, and lethargy. Abnormalities on physical examination included a large irregular mass in the mid-abdomen and left-sided iridial neovascularization with a small white mass on ventral/lateral limbus. On CBC, anemia and lymphopenia were present. Blood chemistry showed mild hypoglycemia, elevated BUN, and elevated AST activity. Patient had a low T4. Survey radiographs showed mild tracheal elevation in the cranial mediastinum, no obvious pulmonary metastasis, normal heart size, and a mass effect in the mid abdomen.

DX

Pancreatic carcinoma with splenic metastasis

Sonographic Differential Diagnosis

Multicentric neoplasia, likely lymphosarcoma or similar neoplastic process.

Image Interpretation

The abdomen in this patient presented multiple hypoechoic lymph node masses involving the spleen, pancreas, and periportal lymph nodes. A mild amount of abdominal fluid was noted. Multiple hypoechoic nodular changes are noted in the area of the left pancreatic limb consistent with lymph nodes or pancreatic nodules. A large hypoechoic pancreatic mass is present. The spleen in the near field is significantly irregular with multiple hypoechoic nodular changes and scalloping capsular contour. The mid field demonstrates an adjacent infiltrated lymph node.

Outcome

A guarded prognosis was given. The patient was treated with steroids and gastroprotectants.

Clinical Differential Diagnosis

Abdominal neoplasia of renal, pancreas, intestine, spleen, or mesenteric origin, lymphoma, abdominal cyst, hydronephrosis.

Sampling

US-guided FNA cytology from pancreas and spleen revealed pancreatic carcinoma and splenic congestion with metastatic tumor cells, respectively.
