This 14-year-old MN DSH presented for partial anorexia, progressively worsening abdominal pain on serial examinations, and weight loss. CBC and blood chemistry analysis revealed a severe lipase elevation with otherwise normal parameters.

DX

Pancreatic carcinoma with moderate atypia and no evident inflammation or necrosis

Sonographic Differential Diagnosis

Focal pancreatomegaly with mild peripancreatic inflammation. The size, echogenicity and focal involvement of the pancreas with lack of severe peripancreatic inflammation is worrisome for neoplasia. Emerging, severe pancreatitis cannot be definitively ruled out.

Image Interpretation

The mid aspect of the left limb of the pancreas is markedly enlarged with lobulated serosal margins. The pancreatic parenchyma is hypoechoic, mildly heteroechoic and moderately well vascularized. The surrounding mesenteric fat is mildly echogenic.

Outcome

The patient was euthanized due to poor prognosis.

Clinical Differential Diagnosis

Pancreatitis- IBD, neoplasia of pancreatic origin, extra-pancreatic neoplasia.

Sampling

US guided 22 and 25 gauge FNA revealed pancreatic carcinoma with moderate atypia and no evident inflammation or necrosis.
