ORTHO – Bilateral tendovaginitis of the biceps tendon, stage I in a 5 year old M Wire-haired Pointing Griffon

This 5 year old M Wirehaired Pointing Griffon presented with intermittent right forelimb lameness of 5 months duration. Moderate spasm right biceps tendon stretch and tension noted in triceps on shoulder extension.

Image Interpretation

Ultrasound of the shoulders –

Left: The biceps tendon is regular in outline and echo pattern. There is scant tendon sheath effusion & synovial thickening. The bone surface of the intertubercular groove is mildly irregular. A small non-reactive synovial hernia/facultative bursa is noted medial
to the tendon sheath next to the minor humeral tubercle and considered incidental.
Mild to moderate periarticular osteophytes are noted as well as mid shoulder joint
effusion.

Right: 

The biceps tendon is well delineated and uniform in echogenicity with a regular echo texture. There is scant tendon sheath effusion with slight synovial thickening.  Mild to moderate periarticular osteophytes are noted as well as mid shoulder joint effusion.

DX

The ultrasonographic findings are compatible with emerging bilateral tendovaginitis of the biceps tendon: the findings are compatible with early stage 1 disease and ultrasonographically more pronounced for the left side.

Outcome

Mild to moderate chronic DJD is noted for both shoulder joints.
A bicipital impingement syndrome by supraspinatus tendinopathy cannot be
determined on either side ultrasonographically.
Conservative management with rest, pain relief, systemic NSAIDs, and local injection
of triamcinolone should be considered. At present surgical revision is not indicated.
