MRI of the head.
The anatomy and symmetry of the brain parenchyma is as expected. Possible mild bilaterally symmetric loss of the hippocampus volume is noted. No structural abnormality is seen.
Possible mild increase in signal intensity of the grey matter of the forebrain is noted with a symmetric distribution in FLAIR and T2weighted sequences. Except for this the signal intensity and contrast enhancement of the brain is within normal limits in all sequences.
There are no overt structural abnormalities of the brain parenchyma. An underlying cause of the clinical presentation is not detected. This does not rule out a number of physical or functional brain disorders including temporal lobe sclerosis or hippocampus atrophy which is a common reason for late onset seizural activity. Past trauma and cerebral microbleeds are another possible underlying cause of geriatric seizure activity. The MRI should be complemented by CSF examination to screen for the unlikely event of a meningoencephalitis.
