The thorax in this patient revealed a lung mass with pleural effusion. The mass measured approximately 4.0 x 3.0 cm. Multi focal mineralization was noted within the lung mass, which would suggest lung base carcinoma. The lung mass appeared to be obstructing the vena cava causing a passive congestion liver pattern. The heart was volume contracted and displaced by the lung mass.
Dilated hepatic veins were noted. This is likely the cause of the free fluid in the abdomen.
Transdiaphragmatic view revealed pleural effusion.
Prognosis is poor. Pleurocentesis or direct FNA of the lung mass is recommended along with oncology evaluation.
