Lymphoma, colonic in a 10 year old MN DSH cat

A 10-year-old MN DSH cat was presented for poor appetite, hematochezia, and weight loss. The cat was anemic on referral blood work. No blood analysis was performed pending sonogram results owing to economic concerns.

DX

Colonic mass-lymphoma.

Sonographic Differential Diagnosis

Annular mass and separate mass in pelvic inlet. Colonic lymphoma, mast cell disease or carcinoma are all possibilities.

Image Interpretation

The upper gastrointestinal tract appeared normal in this patient, however the proximal descending colon presented an annular mass with hypoechoic ill defined thickening at the area where the transverse colon begins to form the descending colon. Obstructive colonic stool pattern was noted. A separate mass was noted in the pelvic inlet in the area of the cystourethral junction and further caudally. These were isolated to the descending colon.

Comments

Subtotal colectomy could be performed, however it may be difficult to completely resect the mass in the distal descending colon.

Clinical Differential Diagnosis

GI tract pathology (neoplasia of the lower intestinal tract/colon – lymphoma, adenocarcinoma, leiomyoma, leiomyosarcoma, mast cell tumor; severe inflammatory bowel disease)

Sampling

US-guided FNA of annular mass and separate mass in pelvic inlet. Cytology results of the colonic mass confirmed a diagnosis of lymphoma.
