A 15-year-old NM Jack Russell Terrier was presented for evaluation of hypercalcemia with suspected suspect primary hyperparathyroidism as total and ionized calcium were high, inorganic phosphate was low, PTH was high normal and rPTH was low.

DX

Left parathyroid adenoma.

Image Interpretation

: Left thyroid in this patient revealed a 0.67 x 0.46 cm hypoechoic nodule at the cranial aspect of the left thyroid lobe; the nodule was demonstrated short and long axis. Right thyroid lobe was uniform, unremarkable measured approximately 0.5 cm in width.

Medial parathyroid was normal at 0.2 x 0.18 cm.

The esophagus was thickened and dilated in this patient. The dilation continued in the full visible view of the esophagus. The causes of regurgitation should be considered.  The thickened wall would be suggestive for chronic esophagitis. Barium swallow may be appropriate in this patient or endoscopy if any clinical signs of regurgitation are present.

Outcome

Esophageal dilation of unknown cause.
Left parathyroidectomy is recommended. Empirical treatment for esophagitis would be warranted.
